I would explain everything I am doing before and as I am doing it, and allow time for questions. I would need to collect identifying information as well as background information on the patient such as marital status and code status and determine if a health care proxy is invoked (most likely it is not since they have not seen a health care provider before) as well as family history. I would need to determine if the patient uses any assistive devices such as hearing aids, glasses or contacts, dentures, walkers, wheelchairs or canes. This would help me determine how to better care for them and inform me on any special needs there patient may have. I would also find out when their last bowel movement was ( to ensure that during their stay with us they don’t require any intervention to love their bowels), when the last time they urinated, any pertinent medical history including psychiatric issues the patient may have concerns about such as anxiety or depression. Is this patient currently taking any medications? Do they have any allergies to medications? How do they prefer to be addressed as, and how do they take their medications if needed? What is their preferred language? How does this patient prefer to be given information? Written, verbal or via demonstration? Does the patient have any questions for me at this time? I would also ensure to perform a cultural assessment to better communicate with the patient. Techniques used to collect data would be using the different phases of the interview process such as the pre-interaction phase to ensure I collect as much data on the patient as possible before speaking with them, and the working phase where I collect data by asking specific questions. During the whole assessment, I would maintain standard precautions and use inspection techniques like palpating, auscultation, and percussion if needed to gather objective data pertaining to the individual body systems.
